A significant amount of arterial hypoxia and hypocapnia is caused by lung injury caused by the SARS-Cov-2 virus. Whether these circumstances caused a change in hemoglobin’s affinity for oxygen is unclear in the literature.

For the study, researchers investigated arterial blood gas analysis (BGA) results for 517 hospitalized patients with COVID-19 whose oxyhemoglobin dissociation curves (ODC) were measured on admission (i.e., before treatment).

Compared to typical normal p50 (pOχ2at 50% hemoglobin saturation) 27 mmHg, 76% had lower standardized p50 (p50s) and 85% had lower in vivo p50 (p50i). P50s and P50i showed statistically significant changes between baseline and values ​​obtained at all these time points in a subgroup of 33 patients with subsequent BGA after 3, 6, 9, 12, 15 and 18 days of therapy. Patients who had p50 values ​​below 27 mmHg (p = 0.012) had a higher chance of survival at admission based on 30-day Kaplan-Meier survival curves of patients with COVID-19 stratified by p50i level.

It is not known whether the altered oxygen affinity of hemoglobin in patients with COVID-19 was a direct or indirect result of the virus’ effect on hemoglobin.
